What's the difference between a serious, willful, and repeat violation?

OSHA classifies a violation as serious when a hazard could cause death or serious harm and the employer knew or should have known; willful when the employer knew of the requirement and intentionally disregarded or showed plain indifference to it; and repeat when OSHA finds a substantially similar violation to a prior citation. Willful and repeat violations carry the highest penalties — up to roughly ten times a serious violation.
